🎯 Target Lesion

Each case contains one target lesion that needs to be assessed. At the start of each case, the target lesion is highlighted with a pulsing orange ring for 3 seconds.

Use the 🎯 Find target lesion button in the right panel at any time to navigate back to the target and replay the marker ring.

Round 1 — Difficulty Rating

Scroll through the axial and coronal slices to assess the tumor and its relationship to surrounding structures.

Please rate from the perspective of an average interventional radiology centre, without advanced navigation tools.

Round 2 — HEPAR Score

Use the Distance tool to measure distances, then assign the correct score range for each item in the right panel.

  • Maximum tumor diameter
  • Shortest distance: tumor margin → liver capsule (subcapsular)
  • Shortest distance: tumor margin → Heart/Lung, Stomach/Duodenum, Other GI organs (jejunum, ileum, colon, appendix), Adrenal gland
  • Shortest distance: tumor margin → Common/lobar hepatic bile duct or gallbladder
  • Shortest distance: tumor margin → Portal vein / IVC / hepatic veins

The total score and difficulty category update automatically: Easy 0-2 · Moderate 3-4 · Hard ≥5
